Leadership in Value-Based Care
Engaging practice leaders to support person-centered, value-based care is essential for building an integrated health system. However, organizational change can be unsettling.
Whether you are developing new workflows, billing procedures, or referral networks; asking staff to learn new skills; implementing new care models; or participating in value-based payment (VBP) arrangements, practice transformation requires more than technical know-how.
Integrated Care DC equips practice leaders to manage change. Currently, 48 managers, executives, and team leaders from 27 primary care, behavioral health, and social service organizations receive training, tools, and peer support through our Leadership Through Change Learning Collaborative. Since 2021, more than 50 District-based practice sites have also leveraged our coaching program to advance their strategic priorities.

Mental Health Matters in May
May is Mental Health Awareness Month, an opportunity to raise awareness about the importance of mental health and reduce stigma around mental illness. The District of Columbia has made progress integrating mental health services into primary care settings through models like Primary Care Behavioral Health (PCBH). However, recent data shows we have more work to do.
The District’s Live.Long.DC. strategic plan aims to leverage health, behavioral health, and housing resources to combat this crisis through a comprehensive strategy.
As Integrated Care DC works to improve residents’ overall health and well-being through team-based, collaborative care, we encourage providers to:

Forging Ahead: Integrated Care DC Enhances Collaborative Efforts in Behavioral Health
A year after nearly 200 behavioral health stakeholders converged for our Managed Care Readiness workshop, Integrated Care DC’s coaching team continues to build on this momentum. The 2023 workshop, which included leadership from the Department of Health Care Finance (DHCF) and the Department of Behavioral Health (DBH), set the stage for integrating behavioral health with primary care, with resources still accessible on our Managed Care Readiness Workshop webpage.
Today, the Integrated Care DC team maintains a regular monthly dialogue with DBH’s State Opioid Response team, opioid treatment providers, and the District’s managed care plans. Our coaching is specifically tailored to help providers excel under these evolving care standards. Additionally, Integrated Care DC is dedicated to continuous learning and sharing, developing new learning series to ensure our coaching remains relevant and impactful.

CRISP DC Unveils Social Determinants of Health (SDOH) Reports
CRISP DC has introduced a new suite of Social Determinants of Health (SDOH) Reports within its PopHealth Analytics tool. These reports, which capture the social needs of Medicaid beneficiaries, allow stakeholders to develop targeted interventions, programs, and policies to address social needs and improve health outcomes.
The SDOH Reports categorize social needs based on definitions from the Gravity Project, including education and literacy, physical environment, homelessness, food insecurity, housing instability, transportation insecurity, financial insecurity, and more. The data are displayed through an interactive SDOH Map and Dashboard, enabling users to visualize the prevalence of social needs by geographic area and to explore beneficiary and claim-level details.
Additionally, CRISP DC has enhanced its Population Navigator Tool and Panel Demographic Report with filters for social needs categories. Users can now examine reports for specific population segments, including those with identified SDOH.

Opioid Abatement Strategic Impact Grant
The DC Department of Behavioral Health’s Office of Opioid Abatement has released a Request for Applications (RFA) for organizations in the District of Columbia to conduct opioid abatement activities based on the fourteen recommended areas of focus made by the Opioid Abatement Advisory Commission. “Opioid Abatement” refers to programs and activities that end or lessen the impact of the opioid epidemic. This grant will provide more tools to address the opioid epidemic. For more details, please visit the Office of Opioid Abatement website.
You can help inform Medicaid beneficiaries about renewing their Medicaid coverage and exploring other available health insurance options (if they no longer qualify for Medicaid, Alliance, or the Immigrant Children’s Program). Find training videos, meeting recordings, and a wealth of information on the renewal process on the DC Medicaid Renewal webpage. DHCF’s Stakeholder Communications Toolkit also has instructions for using the District’s online benefits portal, District Direct. You can help individuals create an account, update their information, and access call centers where interpreters and in-language staff are on hand to assist. All Medicaid beneficiaries should renew their health coverage by or before June 30, 2024.
