Description: This question-and-answer session is an opportunity for those interested in or registered for the Value-Based Care Learning Collaborative to learn more about the assessment/application process and overall expectations prior to enrollment and launch.
Before the session, we encourage individuals to have joined the VBC Learning Collaborative Introductory Sessions and/or have viewed the recording in our resource library and come prepared with questions about the Value Based Care Assessment Tool and Application presented. The brief assessment tool and application is required for enrollment into the VBC Learning Collaborative. We will review and assist with completion and discuss next steps for enrollment into one of three tracks.
