Integrated Care DC Releases At-A-Glance Report

Integrated Care DC’s newly released At-A-Glance report highlights the significant strides participating providers have made as they work to deliver integrated, whole-person care and prepare for value-based payment (VBP) models. Altogether, 39 practice teams from 35 organizations engaged in 1:1 coaching during Year 4 (August 14, 2023 – August 13, 2024). Among the 24 practice sites engaged in at least six months of coaching, 82% scored at or above an intermediate level of integration in Year 4 based on an annual reassessment of their progress across key aspects of integrated care, up from 76% in the previous year.

With the Leadership Through Change Learning Collaborative and Cost of Care Learning Series, the project’s shift to a cohort-based approach in Year 4 enabled participants to progress through content together while exchanging ideas and insights with their peers on enhancing organizational effectiveness and sustainability. Our yearlong Value-Based Care Learning Collaborative, launched on July 30, 2024, provides practice teams in each of three cohorts (Pediatric Care, Maternity Care, and Care of Acute and Chronic Care Conditions) access to group and 1:1 coaching sessions to improve their performance on quality metrics applicable to current VBP programs offered by the District’s Medicaid managed care plans.

In Year 5, Integrated Care DC will continue to deliver quality improvement and value-based care readiness support that pairs group learning with individual practice coaching and peer-to-peer sharing opportunities. The project will also foster linkages between health and social service organizations to address health-related social needs.
