1115 Whole-Person Care Learning Collaborative

Expanding Care and Improving Outcomes

Expand your organization’s impact and financial sustainability by joining the 1115 Whole-Person Care Learning Collaborative. This seven-month initiative is designed to help providers and organizations that deliver or partner with housing, nutrition, and reentry services prepare for Medicaid payment and operations.

Track 1

Housing Supports

This track aims to build participant capacity to implement newly proposed Medicaid housing supports to enhance health outcomes by addressing housing instability among beneficiaries, reducing hospital readmissions and emergency department visits, and increasing access to preventive care, ultimately allowing individuals to stabilize and thrive.

Track 2

Nutrition Supports

This track aims to build participant capacity to implement newly proposed Medicaid services to enhance health outcomes by tackling food insecurity among beneficiaries, ensuring access to nutritious food, reducing diet-related chronic conditions, and improving overall well-being.

Track 3

Reentry Supports

This track aims to build participant capacity to implement newly proposed Medicaid services to improve continuity of care, access to preventive and routine healthcare and case management, and overall well-being for individuals transitioning back into the community.

Who Should Join

This learning collaborative is ideal for organizations that provide – or wish to provide – services in these categories to Medicaid beneficiaries. Organizations do not need to be current Medicaid providers; however, they should be interested in joining this new Medicaid system of care. Our priority audience includes:

  • Community-based organizations (CBOs) and service providers who will be new to the Medicaid program, such as food pantries, meal delivery services, housing agencies, and correctional health/reentry programs.
  • Medicaid providers – primary care, behavioral health, substance use disorder, and other specialties.
  • Justice Involved health and community who will provide 90-day pre-release services, as well as post-release community services to justice involved individuals.

Why Join

Starting in 2026, once approved, the 1115 waiver will provide health-related social needs (HRSN) and reentry service providers with sustained Medicaid funding. The 1115 Whole-Person Care Learning Collaborative will set you up to successfully participate in this next phase of Medicaid innovation, which includes connections to care for the individuals and families you serve. Specifically, participants will:

  • Identify needs and develop infrastructure to be ready for Medicaid payment for newly funded services.
  • Improve workflows for screening and assessment of HRSN and reentry supports.
  • Enhance your ability to refer to other health care and HRSN service providers, giving your clients access to a broader array of services that serve the whole person.
  • Implement strategies to support data sharing within your organization and with partners.
  • Share challenges and best practices with your peers and DHCF, and spread your learnings across your organization.
  • Learn from experts who have implemented 1115 Medicaid transformation in other parts of the country and helped providers participate and succeed.
  • Support goals of reducing health disparities of the District’s justice-involved population and of improving transitions for returning citizens.

What Can I Expect

Participants will commit to attending monthly interactive virtual convenings, tailored 1:1 technical assistance and coaching, and peer-to-peer learning opportunities.

  • January-February: Introductory group learning sessions, enrollment into tracks, and completion of baseline assessments.
  • February-June: Monthly group, peer-to-peer, and individual learning opportunities to support readiness by and between tracks. Topics will include payment and contracting options, implementation plan development, screening and assessment, referral network development and tracking, and service and cost data sharing and reporting.
  • July: Gather and share lessons learned via virtual or in-person session and updates on next steps.

With the training and technical assistance delivered through this Learning Collaborative, participants will gain the capabilities, knowledge, and individualized strategies to initiate and/or expand the provision of the proposed services and receive Medicaid reimbursement.
