Join the 1115 Whole-Person Care Learning Collaborative
Kickoff Sessions: January 21, 2025, 1–2 PM & January 23, 2025, 11 AM–12 PM
Expand your organization’s impact and financial sustainability by joining Integrated Care DC’s 1115 Whole-Person Care Learning Collaborative. This seven-month (January-July 2025) initiative is designed to help providers and organizations that deliver or partner with housing, nutrition, and reentry services prepare for Medicaid payment and operations.

Value-Based Care Learning Collaborative at the Midpoint: Advancing Quality and Outcomes for DC Medicaid Beneficiaries
Twenty-three DC Medicaid provider practice teams are testing changes to address gaps and improve care during the second learning cycle of Integrated Care DC’s Value-Based Care Learning Collaborative. On January 8, 2025, the teams will meet in-person to share their progress, learn best practices for sustaining improvement, and engage with a panel of the District’s Medicaid managed care plans to discuss the plans’ current and anticipated value-based payment programs.
This collaborative is designed to enhance care delivery and performance on quality metrics tied to value-based payment (VBP) arrangements. The year-long initiative comprises four learning cycles: Plan, Implement, Improve and Ingrain, and Sustain and Report. Each cycle provides webinars, individualized coaching, and peer-sharing opportunities to help teams navigate VBP and improve care. After completing an initial assessment, teams develop and conduct quality improvement projects within one of three tracks: Pediatric Care, Maternity Care, or Care of Acute and Chronic Conditions.
Advancing Health Information Technology for HCBS Providers: A Transformative Initiative
The District of Columbia’s Department of Health Care Finance (DHCF) made significant advancements in modernizing electronic health record (EHR) infrastructure for Home and Community-Based Services (HCBS) providers. Funded by the American Rescue Plan Act (ARPA) from 2022 to 2024, the HCBS Digital Health Technical Assistance Program addressed long-standing gaps in healthcare technology adoption among HCBS providers, a group historically excluded from EHR incentive programs. The initiative offered Medicaid HCBS providers tailored technical assistance and ARPA-funded EHR incentives. As a result, 91 organizations have modernized their health record systems and connected with the District’s Health Information Exchange (HIE), addressing the technological gap among HCBS providers traditionally excluded from EHR incentives.
Practice Spotlight: Honoring Individual Power & Strength (HIPS)
Empowering Communities through Integrated Care
For over 30 years, HIPS has supported the health and dignity of individuals affected by sex work and drug use through harm reduction services, advocacy, and peer-led initiatives. As a Medications for Opioid Use Disorder (MOUD) provider and recipient of a Department of Behavioral Health (DBH) State Opioid Response (SOR) grant, HIPS identified an urgent need to enhance its referral systems to better connect clients to behavioral health, primary care, and social services.
CRISP DC Update: Race and Ethnicity Data Now Available in PopHealth Analytics Reports
CRISP DC has enhanced its PopHealth Analytics reports in the CRISP DC HIE Portal to include race and ethnicity data, providing users deeper insights into patient populations.
This new feature mirrors the existing breakdowns by age and gender, enabling healthcare providers to analyze data more comprehensively and identify disparities in healthcare utilization.
