Description: This question-and-answer session is an opportunity for those interested in or registered for the Value-Based Care Learning Collaborative to learn more about the assessment/application process and overall expectations prior to enrollment and launch.

Before the session, we encourage individuals to have joined the VBC Learning Collaborative Introductory Sessions and/or have viewed the recording in our resource library and come prepared with questions about the Value Based Care Assessment Tool and Application presented. The brief assessment tool and application is required for enrollment into the VBC Learning Collaborative. We will review and assist with completion and discuss next steps for enrollment into one of three tracks.

• Receive and complete the brief assessment of your practice’s current and/or potential value-based payment contract(s) to assist in choosing a track for enrollment.
• Confirm your practice’s commitment to participate in the VBC Learning Collaborative.
• Identify and address questions about the VBC Learning Collaborative and associated tracks with peers and the Integrated Care DC Team.

Course Completion & CE Requirements:

Health Management Associates, #1780, is approved to offer social work continuing education by the Association of Social Work Boards (ASWB) Approved Continuing Education (ACE) program. Organizations, not individual courses, are approved as ACE providers.  State and provincial regulatory boards have the final authority to determine whether an individual course may be accepted for continuing education credit. Health Management Associates maintains responsibility for this course.  ACE provider approval period: 09/22/2021 – 09/22/2025.   Social workers completing this course receive 0 continuing education credit.

The AAFP has reviewed the Integrated Care DC webinar series and deemed it acceptable for AAFP credit. Physicians should claim only the credit commensurate with the extent of their participation in the activity. This session is approved for 0 Online Only, Live AAFP Prescribed credits.

Certificates of completion will be emailed within 10-12 business days of course completion.
