Description: This webinar kicks off the Care of Acute and Chronic Conditions track of the “Value-Based Care Learning Collaborative: Improve Quality, Outcomes, and Value in Health Care.” Participants will learn about the importance of team engagement in this project, which team members will be necessary for success, and best practices for engaging a team and keeping them engaged. The presenters will describe basic concepts of quality improvement that will guide the approach to this year-long learning collaborative, including steps to develop a logic model and use it to guide quality improvement plan-do-study-act (PDSA) cycles and measurement. Attendees will be equipped to engage their team in the learning collaborative and start to develop their individualized logic model for improvement of well-care visits for infants and children.
