Series Title: “Control Your Future!”: The Promise and Practice of Advance Care Planning
Series Description: Only about one-third of Americans complete living wills, durable power of attorney for healthcare forms, Psychiatric Advance Directives, or other types of Advance Care Planning to document their wishes for which treatments they are willing to undergo in the event they become incapacitated. Even when they have living wills, those documents may not be implemented in an emergency because they aren’t readily available. In this three-part series, primary care and behavioral health providers will learn behavioral strategies for engaging even reluctant patients in Advance Care Planning and uploading the resulting documents to the health information exchange through CRISP DC’s AD Vault. Case illustrations will be used throughout. Providers will be invited to reflect upon and address their own discomfort with Advance Care Planning.
Description: This interactive webinar will cover several key topics: clarifying the purpose and processes of Advance Care Planning; its importance for patient self-determination and reducing healthcare costs; providers’ roles as trusted guides in engaging patients in planning; and technological solutions for ensuring Advance Directives are widely downloadable when most needed. CRISP DC’s AD Vault will be introduced. Case illustrations will be used throughout.
