Engaging practice leaders to support person-centered, value-based care is essential for building an integrated health system. However, organizational change can be unsettling. Whether you are developing new workflows, billing procedures, referral networks, asking staff to learn new skills, implement new care models, or participate in value-based payment (VBP) arrangements, practice transformation requires more than technical know-how. Navigating uncertainty requires adaptive leadership and a clear communication plan to involve internal and external stakeholders in the change process and to defuse potential resistance.
Integrated Care DC equips practice leaders to manage change. Currently, 48 managers, executives, and team leaders from 27 primary care, behavioral health, and social service organizations receive training, tools, and peer support through our Leadership Through Change Learning Collaborative. Since 2021, more than 50 District-based practice sites have also leveraged our coaching program to advance their strategic priorities.
