Who Should Join
This learning collaborative is ideal for organizations that provide – or wish to provide – services in these categories to Medicaid beneficiaries. Organizations do not need to be current Medicaid providers; however, they should be interested in joining this new Medicaid system of care. Our priority audience includes:
- Community-based organizations (CBOs) and service providers who will be new to the Medicaid program, such as food pantries, meal delivery services, housing agencies, and correctional health/reentry programs.
- Medicaid providers – primary care, behavioral health, substance use disorder, and other specialties.
- Justice Involved health and community who will provide 90-day pre-release services, as well as post-release community services to justice involved individuals.
Why Join
Starting in 2026, once approved, the 1115 waiver will provide health-related social needs (HRSN) and reentry service providers with sustained Medicaid funding. The 1115 Whole-Person Care Learning Collaborative will set you up to successfully participate in this next phase of Medicaid innovation, which includes connections to care for the individuals and families you serve. Specifically, participants will:
- Identify needs and develop infrastructure to be ready for Medicaid payment for newly funded services.
- Improve workflows for screening and assessment of HRSN and reentry supports.
- Enhance your ability to refer to other health care and HRSN service providers, giving your clients access to a broader array of services that serve the whole person.
- Implement strategies to support data sharing within your organization and with partners.
- Share challenges and best practices with your peers and DHCF, and spread your learnings across your organization.
- Learn from experts who have implemented 1115 Medicaid transformation in other parts of the country and helped providers participate and succeed.
- Support goals of reducing health disparities of the District’s justice-involved population and of improving transitions for returning citizens.
