Twenty-three DC Medicaid provider practice teams are testing changes to address gaps and improve care during the second learning cycle of Integrated Care DC’s Value-Based Care Learning Collaborative. On January 8, 2025, the teams will meet in-person to share their progress, learn best practices for sustaining improvement, and engage with a panel of the District’s Medicaid managed care plans to discuss the plans’ current and anticipated value-based payment programs.
This collaborative is designed to enhance care delivery and performance on quality metrics tied to value-based payment (VBP) arrangements. The year-long initiative comprises four learning cycles: Plan, Implement, Improve and Ingrain, and Sustain and Report. Each cycle provides webinars, individualized coaching, and peer-sharing opportunities to help teams navigate VBP and improve care. After completing an initial assessment, teams develop and conduct quality improvement projects within one of three tracks: Pediatric Care, Maternity Care, or Care of Acute and Chronic Conditions.
Highlights of ongoing projects include:
- Pediatric Care: Streamlining appointment scheduling to increase well-child visits.
- Maternity Care: Addressing obstetric appointment no-shows and enhancing hypertensive patients’ postpartum monitoring.
- Acute and Chronic Conditions: Leveraging behavioral health supports to boost patient engagement and addressing preventable hospitalizations.
Throughout the first two cycles, teams established improvement frameworks, identified barriers, and implemented SMARTIE (Strategic, Measurable, Ambitious, Realistic, Time-bound, Inclusive, and Equitable) goals using Plan-Do-Study-Act (PDSA) cycles to test changes. Participant feedback to date underscores the value of collaboration, insights from Medicaid managed care plans, and practical strategies to overcome challenges.
As the collaborative progresses into its next phases, participants will focus on sustaining initiatives and expanding learnings to other quality metrics. Results and lessons learned will be showcased at a final event in June 2025.
